Differential diagnosis The other holosystolic murmurs are associated with Tricuspid regurgitation and Ventricular  Septal Defect. Midsystolic ejection murmurs such as with Aortic and Pulmonary Stenosis may be confused with holosystolic mumurs, and in listening to the murmur, attention should be paid to the time interval immediately following the first heart sound.
